    How to Obtain Copies of Voter Checklist The Supervisors are required to provide copies of the Checklist to those who request it. Supervisors are urged to address all pending items prior to providing copies of the Checklist. However, the the Right-to-Know law requires that a public record that is immediately available be provided immediately. A person making a request for a copy of the Checklist should not be denied because there are pending changes, but they should be informed that the Checklist is not current. A request received by mail should not be unreasonably delayed while corrections are made. The Supervisors may only provide Checklist information for their own town or city. The Supervisors shall charge a fee for each copy of the public checklist for the town plus any shipping fees if applicable. The information on the Checklist that Supervisors of the Checklist may provide upon payment, include the voter's name, street address, mailing address, town or city, voter history for an election (including whether or not they voted absentee), and party affiliation, if any, of every registered voter in the town or city. Date (or any date range) of a voter's registration is not public information. The Supervisors of the Checklist may provide public Checklist information on paper, or electronically. **It is a crime to knowingly use the Checklist information for commercial or purposes such as selling or offering for sale any property or service unrelated to an election or political campaign. *To request a copy of the Voter Checklist to purchase, you can either print the form that we have provided for you in an attachment below, or come into the office and get a form to fill out and sign. *The Voter Checklist is a public record and therefore is open to view in our office during regular business hours.     Acworth NH Vital Records Vital Records Effective January 1, 2005, all individuals requesting a certified copy of a record (Pursuant to RSA 5 C-16 ) must present positive identification, including, but not limited to, a driver's license, passport or other picture identification, or in the absence of acceptable picture identification shall complete the documentary evidence form . To obtain a certified copy of your marriage certificate you can come in to the office Wednesday, 4:30 pm - 7:00 pm or Saturday 8:00 - noon, and with proper picture identification you can apply for, and with a short wait, obtain a copy of a vital record certificate. The Town Office is located at 13 Town Hall Road. Or, you can apply for vital records by mail. You must print and complete our Order Form, include positive identification, including, but not limited to, a PHOTOCOPY of your driver's license, a PHOTOCOPY of your passport, or a PHOTOCOPY of other picture identification, or in the absence of acceptable picture identification shall complete the form State of NH Documentation Evidence for individuals NOT possessing an acceptable picture identification. Enclose this with your check made payable to: Town of Acworth , along with a self-addressed stamped envelope and mail to the Acworth Town Clerk, PO Box 37, Acworth, NH 03601. A fee of $10 is required by law in the State of NH for each subsequent copy issued at the same time as the initial copy. Vital records are considered private and confidential, with access limited to those individuals who have a "direct and tangible" interest in the record. The only exceptions are records prior to 1948, which are considered public. Only the registrant, a member of his/her immediate family, or a representative, may obtain copies of vital records or someone with a "direct and tangible interest" such as an attorney. In order to determine if the "legal representative" may have access to the record, the direct and tangible interest must be listed in any written correspondence with a copy of such legal document being provided. Those notes will be published to the town newsletter and town webpage. Announcement Feb 22, 2023 – Small Roads Update The process of reviewing and collating the dozens of questions and topics that were raised is in progress. Many questions overlap and most of the questions came at the public hearing, which our town administrative assistant recorded. The team is going through the 3 1/2-hour recording and transcribing it. It’s important to know that many of the topics require additional research and some require land-owner inquiries. The town office is fully engaged now with other priorities; end of year actions and preparations for the town meeting, etc. Their time is at a premium this time of year. The project team met and agreed to address all the questions received together in one response. It will take time and support from several resources to complete that effort. It’s estimated that it will take most of the month of March to complete the reviews, the additional research, reach out to individual landowners (as required) and to draft the responses. In April the responses will be posted to the town web page, and we will announce it in the Select Board meeting minutes (which are also posted in the town newsletter). Revised individual road assessments and updated FAQ's will be updated in the April timeframe. If anyone is interested to engage in this effort, the meeting schedule is posted on the town webpage. Meeting minutes will be provided on the web page and referenced in the Select Board meeting minutes which are also published in the town newsletter. Thank you for your patience and continued support.
